In first ep of the first season and maybe another one there was a lot of that fake humping, but that's about it. Most of it was really compelling stuff that I highly enjoyed. The first season introduced us to two college friends who were the owners of a successful plastic surgery practice. They were both contrasting characters. One was a genius at his work, with a unstable yet close family, while the other wasn't as good at his work but was extremely charismatic and used his 'wiles' to attract business.
Early on in the show the charismatic one took on a case to perform a complete facelift for a lot of cash without asking any questions. Later it turned out the person who they were operating upon was a child molester who had victimized a drug lord's daughter.
The child molester was eventually dealt with, but the drug lord started blackmailing them into doing unethical stuff. Eventually he ended up threatening the 'genius' doctor's family.
While both doctors were in principle good people, the more charismatic one would bend the rules often while the brilliant doctor was far more restrained in other aspects of his life. The show is all about these two characters and how they change and learn after every crisis. By the end of the first season the less flamboyant doctor, who had always shown far less passion shows a lot of charisma when his family was threatened. While this was the meat of the first season, there were lots of other interesting things happening.
This constant struggle with morale issues is what made the first season awesome. You felt like characters were growing and changing every episode.
In the second season it seems like the producers thought,"You know, we need more fake humping and less humanity."
The second season has at least one fake humping moment every episode, and is all about porn stars, marital fidelity and all this other Jerry Springer crap. I am highly disappointed.
